Works as part of a homeless outreach team to engage homeless individuals and families in the Region 1 and Region 6 areas of Oahu. Screens clients for appropriate housing programs, oversees/conducts the application process, and conducts housing searches. Assists clients to acquire housing, identify intervention recommendations through an individual service plan, and ensure linkage with support team to assist with maintaining housing and improving quality of life. Utilizes housing-focused outreach and harm reduction approaches when working with clients.
As part of the homeless outreach team, locates individuals and maps their locations throughout the assigned regions.
Engages with individuals and families to build a quality working rapport, providing information, improving access to mainstream services and facilitating the process of securing housing for individuals who want housing.
Meets with clients, assesses and determines the appropriate housing to resolve their housing needs.
Provides resources and referrals to individuals and families that assist with problem solving and address immediate needs.
Conducts housing searches and builds inventory lists of available housing units for candidates. Liaises with property managers, landlords, realtors and other sources to identify and secure vacant properties for client placement.
Assertively engage with clients to secure housing including unit rental, damage deposits, securing income assistance if required, settling up of utilities, signing of leases, and arranging of furnishing and household items to establish their homes.
Makes appointments for clients and escorts them to such appointments as needed.
Provides back-up and coverage for other outreach workers when necessary.
Works in collaboration and consultation with other homeless outreach programs and personnel as part of a seamless continuum of service.
Advocates on behalf of clients to ensure equity of treatment.
Adheres to HCHP Outreach Guidelines.
Meets service targets as identified in the contract.
Tracks demographics of clients and all of data required in the contract.
Keeps records of all client cases, and is responsible for ensuring that the records are entered into the clinic computer data-base, and for safeguarding clients' confidentiality and other rights.
Works with other Project and KPHC staff in providing a coordinated system of care to the clients.
Works as part of a team with other staff for purposes of outreach, back-up, and shared service delivery.
Assists other Project staff in obtaining information on clients from other agencies.
Perform other related duties as assigned
Observe safety and security procedures; determine appropriate actions beyond guidelines; report potentially unsafe conditions; use equipment and materials properly.
Regular and reliable on-site attendance is required
