Primary Purpose:

Assesses the needs of homeless individuals and families in Regions 1, 4, and 6 of Oahu through a standardized assessment tool (VI-SPDAT). Assists them to acquire housing, identify intervention recommendations through an individual service plan, and ensure linkage with support team to assist with maintaining housing and improving quality of life. Utilizes housing-focused outreach and harm reduction approaches when working with clients. Collaborates with contracted partners to ensure that services are being provided and that contract goals are being met.

Essential Duties:

                     Locates individuals and maps their locations throughout the assigned regions.

                     Provides assistance, as needed, to Level II and Level III Outreach Workers by partnering with them, either individually or as a group, on all outreach activities.

                     Provides services to lower acuity clients with less complex needs.

                     Engages with individuals and families to build a quality working rapport, providing information, improving access to mainstream services and facilitating the process of securing housing for individuals who want housing.

                     Makes appointments for clients and escorts them to such appointments as needed.

                     Collaborates with community stakeholders such as HPD, the Sherriff's Department, emergency medical services, hospital emergency departments and emergency homeless shelters.

                     Assists clients in acquiring emergency supplies, including food, clothing, bedding, medicine and hygiene items.

                     Assists clients in applying for entitlements and obtaining identification. Where needed, takes clients through entitlement, medical, and mental health systems to connect them with services and benefits.

                     Assists clients with legal entanglements.

                     Conducts intake and assists in assessment, counseling, and referrals for health and social services.

                     Provides counseling, consultation, and crisis intervention as needed. Encourages medication adherence as appropriate.

                     Assists with family and social relationships as appropriate. Where needed, teaches communication and social skills development.

                     Provides interim case management services for each client which include:

-          outreach / engagement / housing needs and home visits

-          preparing a plan, based on clients' needs and goals, for the provision of services

-          meeting with clients on a regular basis

-          following up with clients and providers of services to determine outcome of linkages

-          providing psycho-social rehabilitation services (teaching hygiene and self-care, housing skills, budgeting, problem-solving, etc.)

-          reviewing treatment plans at least every six months

                     Provides back-up and coverage for other outreach workers when necessary.

                     Works in collaboration and consultation with other homeless outreach programs and personnel as part of a seamless continuum of service.

                     Advocates on behalf of clients to ensure equity of treatment.

                     Adheres to HCHP Outreach Guidelines.

                     Meets service targets as identified in the contract.

                     Tracks demographics of clients and all of data required in the contract.

                     Keeps records of all client cases, and is responsible for ensuring that the records are entered into the clinic computer data-base, and for safeguarding clients' confidentiality and other rights.

     ??               Works with other Project and KPHC staff in providing a coordinated system of care to the clients.

                     Works as part of a team with other staff for purposes of outreach, back-up, and shared service delivery.

                     Assists other Project staff in obtaining information on clients from other agencies.

 

Regular and reliable on-site attendance is required.

Other Duties:

                     Perform other related duties as assigned

                     Observe safety and security procedures; determine appropriate actions beyond guidelines; report potentially unsafe conditions; use equipment and materials properly.

Minimum Qualification Requirements:

 

Skills/Knowledge:

  • Good crisis intervention / de-escalation skills.
  • Ability to relate effectively to difficult clients, including mentally ill, chemically dependent, and other homeless individuals.
  • Ability to operate a vehicle with a valid State of Hawaii driver's license.
  • Must provide proof of automobile insurance and clean drivers abstract.
  • Ability to work independently in an effective manner.
  • Strong communication and interpersonal skills.
  • Ability to work as a team player.

 Education:

  • High school diploma or GED equivalent.

 Experience:

  • One or more years providing direct services to homeless persons preferred.
  • Knowledge of community resources and familiarity with Central Oahu, Kakaako, Waimanalo, and Downtown/Chinatown areas preferred.
  • Training in substance abuse / dual diagnosis preferred.
